Appointments to see Dr Carroll for clinical consultations are by referral from a General Practitioner or occasionally by another Medical Practitioner. You should note the following:

• A referral must be in the form of written communication and dated either on the day or before the day of the consultation. It may be forwarded in advance by either standard mail, facsimile or E-mail. Your doctor may prefer to have you collect the referral from his/her surgery and bring it with you when you come for the consultation. When you attend for a consultation please bring along with you any X-rays, scans or results that you may have as these are very useful and may save you needless repetition.
• Referrals are valid for 12 months from the day of your first visit if written by a GP, but only for 3 months if written by a specialist.
• Medicare will only contribute significantly to the cost of the consultation if you have a valid referral.

• Appointment times are as follows:
  

  Mon	8-40 to 12-20 pm
  Tues	8-40 to 4-00 pm
  Wed	8-40 to 12-40 pm
  Thurs	9-00 to 12-20 pm
  Fri	8 -40 to 4-40 pm
  Sat (1st of the month only)	8-00 to 1-00 pm

  Exceptions can be made for appointments late in the day, but there may be some delay.
